Trip Cancellation Form

The following form is for clients who have already booked a trip with Little Palm Travel (Agency) and for various reasons will have to cancel thier trip.

As usual, we have always recommend vacation protection for our clients. When applicable you may need to submit a claim under your travel insurance policy to avoid additional fees.

If you have need to cancel your trip, please submit the form below granting permission for Little Palm Travel to cancel your trip on your behalf with the supplier.

I understand that this Consent may be executed through the use of an electronic signature in accordance with the Electronic Signatures in Global and National Commerce Act (the “E-Sign Act”), Title 15, United States Code, Section 7001 et seq.), the Uniform Electronic Transaction Act (the “UETA”)), and any applicable state law, and that any electronic signature shall be deemed an original signature for purposes of this Consent, with such electronic signature having the same legal effect as an original signature.
